First Visit
Consultation
Your first appointment to our office is generally a consultation appointment. At this appointment you will have the opportunity to find out more about us as well as your condition, including options for treatment and the risks and indications of various treatment options. This is also an important opportunity for us to evaluate your needs as well as your general health and medical history. Pre-operative instructions, pre-operative medications and treatment plans with cost estimates will be given to you at that time. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Additionally, any procedures to be done under sedation will always require a pre-operative consultation.
If you wish to use dental or medical insurance to offset your cost, please bring your insurance information to this appointment. You will be responsible for obtaining a primary care referral ahead of time if your plan so requires. You will also be responsible for payment of your first appointment, however we will help to file your insurance and either refund any insurance credit or apply it to your treatment plan. Please remember to bring the following information to this very important visit.
Bring to your first vist:
- Current medications
- Your doctors names and phone numbers
- A parent or legal guardian if you are Under 18
- Identification
- Insurance information
- Your surgical referral slip and any X-rays if applicable
X-Rays
If your dentist or physician has taken x-rays, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility. We use all digital x-rays which require a lower dose of radiation and are environmentally friendly.
